Make program selection form fields visible in the instructor portal on the enrollee page.

Currently, the custom program selection form fields are only visible in the instructor portal on the classes page. This means that instructors have to know how to navigate to a specific student in order to see answers to questions like "This student has permission to check themselves out of the program". It would be helpful if these fields were visible on the enrollee page in the portal, so that instructors can navigate there quickly from the In/Out page or the Activities page.
